My transplant coordinators got me in touch with XDX (866-939-2945) and I had to fill out a paper (I don't remember exactly what type) and they sent it to my insurance company I believe. I never payed anything. Since I am at my 5 yr. mark, I don't have any more biopsies or allomapping!!!
